Job Title: Waiver Service Coordinator
Location: Community-based (in-home visits)
Job Type: Full-Time (37.5 hours/week)
Pay Range: $47,000 – $66,000 (commensurate with experience)

What You’ll Do

  • Empower clients through personalized care planning and service coordination
  • Conduct home visits and phone check-ins to assess well-being and eligibility
  • Collaborate with interdisciplinary teams to ensure seamless care transitions
  • Educate and support caregivers in navigating health and community resources
  • Maintain accurate documentation and uphold program compliance standards

 

What You Bring

  • Education: Bachelor’s degree in Social Work, Psychology, Gerontology, or related human (social) services field
  • Experience: At least one year working with individuals with disabilities or chronic conditions
  • Preferred: Experience with electronic documentation and case management systems
  • Skills: Strong communication, critical thinking, and organizational abilities
  • Knowledge: Familiarity with community resources in Southwestern Ohio
  • Requirements: Valid driver’s license and reliable transportation to complete community visits across COA’s service area including Hamilton, Butler, Warren, Clinton, and Clermont Counties.
