Job Title: Aging & Disability Resource Navigator
Location: Office-based in Blue Ash (rotating schedule; outreach required)
Job Type: Full-Time (37.5 hours/week)
Pay Range: $44,600 – $53,890

At COA, Aging & Disability Resource Navigators are the first voice of support for older adults and individuals with disabilities navigating complex care decisions. You’ll provide compassionate guidance, connect callers to life-changing resources, and help families make informed choices—all while enjoying a strong sense of purpose and the support of a collaborative team.

 

What You’ll Do

  • Assist, educate, and support callers from COA’s five-county region as they explore long-term care options
  • Complete telephone and computerized screenings to determine eligibility for COA-administered services
  • Refer individuals to COA programs and community agencies as appropriate
  • Support callers in crisis situations and follow COA protocols
  • Maintain current knowledge of COA programs, Medicare, Medicaid, and long-term care topics
  • Provide education on Medicare Part D options
  • Participate in at least two community outreach activities per month
  • Maintain accurate documentation and uphold HIPAA and confidentiality standards
  • Obtain and maintain certifications (e.g., OSHIIP, AIRS) as recommended

 

What You Bring

Education & Experience

  • Bachelor’s degree in a social services field preferred
  • Two years of relevant work experience may substitute for each year of college
  • At least one year of experience in healthcare, community outreach, medical social work, geriatrics, or long-term care navigation

Skills & Abilities

  • Strong understanding of aging-related issues and community resources in Southwestern Ohio
  • Empathetic, tolerant, and skilled in interviewing and communication
  • Ability to adapt to change and build cooperation among team members
  • Proficient in Microsoft Office, data entry, and multiple software systems
  • Ability to travel within Ohio for training
