The Navigator Team (shared caseload) provides clinical services, congruent with the Recovery Model, that facilitate integration of Behavioral Health and Physical Health services. The Clinical Navigator will complete health education, facilitate communication between Physical Health and Behavioral Health practitioners, attend appointments with individuals, participate in aftercare planning for both Psychiatric and Physical health inpatient stays, and triage complaints to the appropriate level of care.
