Community Outreach Counselor
Provides outreach, engagement, support, and counseling to adults with a serious and persistent mental illness transitioning from inpatient hospitalization to more independent community living. Teaches, supports and assists with activities of daily living/functional performance including: money management, daily scheduling, community integration, medication management, and self-advocacy. Assists clients with maintaining ongoing contact with care providers, following through with appointments and treatment recommendations and informing appropriate providers of outcomes. Must be able to meet with clients in their home environment and in a variety of community settings. Position requires daily travel within Monroe County and occasional travel across six county region. Position requires occasional transportation of clients.
High school diploma and 3 year human services experience or Bachelor’s degree in human services or equivalent combination of education and experience. NYS motor vehicle license, safe driving record and availability of personal vehicle for work required.
Full-time, 35 hours. Schedule will fall within program hours 7:00 AM – 7:00 PM, 7 days a week.
