Community Based Treatment (CBT) Social Worker / Child & Family Treatment Support Services (CFTSS) Behavioral Health Service Provider II
This position will primarily be based in the Allegheny and Wyoming counties however may have cases in Chautauqua and, Cattaraugus counties.
SKILL, QUALIFICATIONS, KNOWLEDGE, ABILITY & PHYSICAL REQUIREMENTS: Minimum Education and/or Professional Qualifications/Skills
Bachelor’s degree in a human service field plus two years of related experience, OR Registered Professional Nurse plus one year of experience,
OR Licensed Occupational Therapist, or Licensed Creative Arts Therapist.
POSITION SUMMARY: The Social Worker/Behavioral Health Service Provider II provides in-home and community-based services to Medicaid enrolled youth ages 0-21 under the Medicaid State Plan service Community Psychiatric Supports and Treatment (CPST) and Social work services through contracted DSS. This position provides services that are goal-directed supports and solution-focused interventions intended to address challenges associate with a behavioral health need and to achieve identified goals or objectives as set forth in the child’s treatment plan. CPST is a multi-component service that consists of therapeutic interventions such as counseling, as well as functional supports. Activities provided under this position are intended to assist the child/youth and family/caregivers to achieve stability and functional improvement in daily living, personal recovery and/or resilience, family and interpersonal relationships in school and community integration. This position will identify resources, make referrals, help with transportation arrangements, identify barriers to effective utilization of services and design strategies to overcome such barriers.
