Conducts initial comprehensive assessments and ongoing assessments of service needs, develops treatment plans forpatients and recommends modification to treatment plans as needed. Maintains patient records and ensures documentation requirements are met.Works well with youth and families and can be part of an interdisciplinary treatment team
Minimum Education and/or Professional Qualifications/Skills:
NYS Education Department license in Social Work, Mental Health Counseling, Psychology, or Marriage and Family Therapy
Experience in a community-centered mental health setting is preferred
Demonstrated cultural competence in understanding and working with disadvantaged/minority populations in community-based programs in anurban and suburban setting
Support overall Agency mission, vision, policies, goals and objectives
Must have a working knowledge of Children’s Services and the healthcare system.
