Job Title: Licensed Assessor (LSW, LPN or RN)
Location: Community-based (with required client visits and in-person meetings)
Job Type: Full-Time (37.5 hours/week)
Pay Range: $45,000 – $64,000

As a licensed social worker or nurse, you’ll conduct person-centered assessments, educate families, and connect individuals to services that preserve dignity and independence—all while enjoying the support of a mission-driven team.

 

What You’ll Do

  • Conduct person-centered assessments to determine eligibility for programs and services
  • Provide education and support to individuals, caregivers, and legal representatives about available resources
  • Advocate for clients, especially in situations involving health and safety risks
  • Report abuse or safety concerns in accordance with COA policies and procedures
  • Maintain accurate and timely documentation in electronic systems

 

What You Bring

  • Ohio licensure as a Social Worker (LSW), Licensed Practical Nurse (LPN), or Registered Nurse (RN) in good standing
  • At least one year of experience in medical social work, home health care, and/or geriatrics
  • Prior experience completing electronic documentation (preferred)
  • Strong understanding of aging-related issues and community resources
  • Proficient computer skills (Windows OS, MS Word, MS Excel, data entry) and ability to learn new software
  • Respect for the cultural, spiritual, racial, and ethnic beliefs of others
  • High-speed internet access at home for secure system access
  • Valid driver’s license and reliable transportation for travel within assigned counties
  • Willingness to travel within Ohio for training
