Patient Information Coordinator

Job Description

This position is to be scheduled 7am to 3:30pm Monday thru Friday with weekends and holidays being scheduled on a rotating basis

The Patient Information Coordinator (PIC) is to provide administrative support for our hospitalists so the clinicians can maximize their time spent on patient care.

POSITION RESPONSIBILITIES

  • Supporting several physicians on an administrative level
  • Properly distribute patients between providers
  • Create daily rounding list
  • Meet with each physician daily
  • Data entry of patient information for billing
  • Distribute final rounding list to clinicians, floors, and nursing stations, etc.
  • Patient Visits – ability to walk to each room/visit every patient every day (required)
  • Distribute brochures explaining how policies of working with PCPs
  • Bedside Patient Experience (PET) questions prior to discharge (required)
  • Assist with PCPs appointments, if and when necessary
  • Gather necessary documents from Medical Records (EMR)
  • Verify insurance, verify documents faxed to PCP
  • Liaison between hospitalists, hospital staff, and Corporate offices
  • Administrative duties as assigned. Schedules, note taking, meeting set up etc...
  • Ability to provide Extended Touch services (required extended patient visits)
  • Extended Touch Reporting, i.e. HCAHPS, Detail Doctor reports
  • Interact positively with providers providing feedback
  • Communicate with management; department and/or one-on-one meetings
  • Provide quality emails, and conference calls.
  • Acknowledgement to emails in a timely manner, within 24 - 48 hours
  • Additional duties as assigned

POSITION REQUIREMENTS, KNOWLEDGE, SKILLS, AND ABILITIES

  • Experience as a Medical Assistant, Unit Coordinator, ER Tech, or similar position
  • Computer literacy
  • Accurately input data entry information (upload documents, fax/electronic submissions)
  • Proficiency in medical terminology
  • Familiarity with ICD 10 and CPT codes (preferred)
  • Ability to interact with patients and physicians
  • Ability to prioritize and have strong problem-solving skills
  • Organizational and Attention to Detail skills
  • Excellent Customer Service – Positive attitude, ability to get along with others
  • Ability to maintain strict confidentiality and security of private and sensitive information
  • Ability to follow HIPAA guidelines/compliance.

PHYSICAL REQUIREMENTS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

  • Ability to walk throughout the facility, stand for extended periods of time and provide handwritten notes.
  • Prolonged standing or walking up to 50% or more of the work day- Typing/writing 75% of the day (required)
  • Occasionally required to stoop, kneel, crouch, or crawl.
  • Frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
  • Specific vision abilities required by this job include close vision, and ability to adjust focus.
  • Frequently required to reach with hands and arms.
  • Regularly required to sit; use hands to handle or feel and talk and hear.
