Neighborhood Self Help by Older Persons Projects (SHOPP) is a non-profit social services agency located in the Bronx serving minority older adults. SHOPP’s mission is based on the philosophy of self-help and mutual assistance. We believe that older people are capable of helping themselves and helping others improve their capacity to deal with the physical, emotional, social, and financial challenges they face as they age.  

 Job Description

The purpose of the Neighborhood SHOPP Case Management Program is aimed at maintaining and assisting homebound older adults to live independently in the community and reducing social isolation. Utilizing a strengths-based approach, Case Management objectives include identification of needs and capabilities through a comprehensive intake and assessment process, determination of eligibility of DFTA-funded in-home and social adult day services, home-delivered meals and other supportive services; monitoring, advocacy, linkage to community services and resources, follow-up on referrals and service provision.  The Case Worker reports directly to the Program Director.  The Case Worker is responsible for the provision of case assistance services to SHOPP’s client’s utilizing a team approach.  The position requires a Baccalaureate Degree in Social Work from an accredited school and experience in a social service setting.  Knowledge and experience with the older adult population and community-based services are also preferred.  Essential competencies, knowledge, and skills include advocacy, brokerage, care coordination, communication, problem-solving, collaborative teamwork and a dedication to empowerment and strength-based social work practice.  The ability to work with a diverse population and Bi-lingual skills in Spanish is helpful.  Computer literacy is mandatory, and knowledge of DFTA Peer Place/ STARS is also helpful.  The Case Worker will provide support to the three teams of case managers.  The case worker will be responsible for direct services to clients and will be expected to conduct home visits and produce CM units.

The Case Worker is specifically responsible for the following:

 Provide Benefits and Entitlement Assistance and Support to SHOPP CMP clients;

  • Screen clients for benefits, entitlements, and services and assess need and eligibility;
  • Assist clients in applying for needed benefits, entitlements, and services and follow up to ensure services are obtained; provide supportive contact as needed;
  • Provide information and referral. To other appropriate agencies as needed and identify community resources for consultation and referral;
  • Work as part of a team focusing on collaboration, cooperation, and shared staff roles and case assignments;
  • Be involved in a supervisory relationship with the Program Director and accept peer-to-peer supervision as well;
  • Conduct self-assessments for annual performance evaluations;
  • In collaboration with team, develop work/off-site schedules;
  • Request leave time and petty cash in a timely manner;
  • Assist team with case record chart management and compliance;
  • Maintain program records and reports required for documentation and statistical reporting for funding sources;
  • Conduct outreach, informational and program presentations to community groups; represent SHOPP at interagency coalitions and professional venues;
  • Maintain and update agency’s Resource Directory.;
  • Provide information and referral to other appropriate agencies and identify community resources for consultation and referral;
  • Participate in the planning and implementation of staff meetings, staff training, staff development and other duties as assigned;
  • Assist with community public relations and promotion of SHOPP and SHOPP events;
  • Perform other tasks as assigned by the Program Director.
