Overview

The Phoenixville Hospital Clinical Pharmacist team consists of well-trained, highly-committed pharmacists who work collaboratively with physicians to meet the needs of adult, pediatric, and neonatal patients.

The pharmacy practice model employed within Phoenixville Hospital is an integrated model consisting of team-based services.

Responsibilities

The Staff Clinical Pharmacist validates accuracy and appropriateness of medication orders, compounds medications and accurately provides prescribed medications
according to professional standards, practices and specific indications or orders. Provides consultative services to internal and external system constituents
regarding medications, interactions, and other drug related information. All Clinical Pharmacits participate in our Pharmacy To Dose. regarding antibiotics and anticoagulation therapy program. Documents
clinical activities and applicable outcomes. Achieves and maintains satisfactory working relationships with pharmacy leadership, pharmacy staff and other
hospital departments, committees and administrators. Places a strong emphasis on patient care with the patient as the highest priority and central focus in all
endeavors and undertakings of the individual and the department.

Qualifications

EDUCATION REQUIREMENTS
- Residency or Pharm D; will accept Bachelor's Degree in Pharmacy with equivalent experience in lieu of residency or Pharm D requirement.

CERTIFICATION AND LICENSURE REQUIREMENTS
- PA PHARMACIST LICENSE

- Board eligible for Pennsylvania licensure may be accepted in lieu of actual license; PA Pharmacist License required with 4 months of hire.

REQUIRED SKILLS
- Excellent Communications Skills, Excellent Interpersonal Skills, Multi Line Telephone Operational Skills, Computer Skills to include accurate and efficient data entry, opening & utilizing multiple programs, searching & understranding drug information databases, utilizing email, navigating intranet and internet and other computer programs as indicated.
