The Case Manager promotes quality of care, supports effective resource utilization, and facilitates the patient’s progression of care to proactively plan for the patient’s safe and timely discharge to the most appropriate level of care. The Case Manager also serves as the key resource for the patient’s multi-disciplinary care team, and works collaboratively with all members of the patient’s health care team and patient representatives to achieve desired clinical, financial and quality outcomes. In addition, the Case Manager analyzes current systems and variances to prevent delays and identify opportunities for improvement.
